Each item has an index number that identifies it in any game. The following list enumerates them according to the way they are stored in the Generation VI Nintendo 3DS games.
Since Generation IV, items may be introduced at new or unused indices, but they are never removed.
List
Rows highlighted in red indicate items introduced in Pokémon Omega Ruby and Alpha Sapphire (or exclusive to it, if next to a row with the same index), while rows highlighted in blue indicate items exclusive to Pokémon X and Y.

Changes
Coming in to Generation VI, many items were changed. Many names were changed to use proper, full spellings. Many sprites were also changed, often because they had not been present since before Generation IV (when items stabilized but some unused items were dropped).
The name of the Bicycle/Bike also changed within Generation VI (seen above), as did the sprites of the Blue Orb, Red Orb, Dowsing Machine, and Mega Charm.
