- The Pokédex generally also registers gender differences (besides those included in the above table) and Shiny Pokémon as separate forms to some degree, though these fail every other metric in the above table. Starting in Gen 7, these are not given distinct form names, except for gender differences in Legends: Z-A. In Gen 4-6, all gendered Pokémon are given the form names "Male" and "Female", even those without gender differences.
- In the Pokédex in Sun and Moon and Ultra Sun and Ultra Moon, Spinda's spot patterns are considered a gender difference. No other source considers Spinda's spot patterns to be any kind of form difference, though they are included in the "Pokémon 1008 ENCOUNTERS" video.
- In the above table, Legends: Arceus and Legends: Z-A are not counted for the menu sprites column, due to these games having drastically different menu sprites from the other core series games which are different for every Pokédex-recognized form (including Shinies and gender differences).
- The Pokémon Guidebook in HOME uses exactly the same form names for every Pokémon as the Pokédex in HOME despite having some differences regarding which forms it includes, resulting in a few oddities: Unfezant, Frillish, Jellicent, and Pyroar have no form names despite having separated form entries, and Genesect has a form name despite its alternate forms not being acknowledged.
- Brilliant Diamond and Shining Pearl does not have different Pokédex entries for forms that had them in previous games due to most of its entries being reused from Diamond and Pearl.
- In the Pokédex in Bank and HOME, some forms have entries for games they do not actually appear in, which are identical to the entries for the Pokémon's regular forms. The forms this applies to are all Mega Evolutions introduced in Omega Ruby and Alpha Sapphire (which have entries for X and Y), Kyogre and Groudon's Primal Reversion forms (which have entries for X and Y), Xerneas's forms (which both have entries for Legends: Z-A), Zygarde's 10% Forme and Complete Forme (which have entries for X, Y, Omega Ruby, and Alpha Sapphire), and Hoopa Unbound (which has entries for X and Y). Additionally, the alternate forms of Arceus, Genesect, and Furfrou have entries for X and Y despite not being included in the Pokédex in that game.
- Partner Pikachu and Partner Eevee have different menu sprites but are not recognized in the Pokédex.
- Cosplay Pikachu is only recognized in the Pokédex in Omega Ruby and Alpha Sapphire. It also has a different menu sprite. Its five costumed forms, despite not being included in the Pokédex, have their own official art and menu sprites.
- Spiky-eared Pichu is only recognized in the Pokédex in HeartGold and SoulSilver.
- The Pokédex in the mobile version of HOME considers Greninja with Battle Bond and Rockruff with Own Tempo distinct forms, though it does not give them form names. Battle Bond Greninja has no Pokédex entries, while Own Tempo Rockruff only has entries for Sword and Shield (which are identical to regular Rockruff's entries).
- Floette's Red Flower form is not given a form name on the Japanese and English Pokédex websites, while its Eternal Flower form is.
- The Sitting Cuties line also includes Zygarde Core and Zenith Marshadow.
- The Pokédex in the mobile version of HOME and the Japanese Pokédex website split Gigantamax Toxtricity into two forms, one for the Amped Form and one for the Low Key Form. Despite this, the Pokédex in HOME uses the same "Gigantamax" form name for both of them.
- Koraidon and Miraidon have form names in the Pokédex ("Apex Build" and "Ultimate Mode" respectively) despite not having any alternate forms that can be registered. However, their ride forms (Koraidon's Limited Build and Miraidon's Low-Power Mode) do have their own menu sprites and unused Pokédex entries and are included in the "Pokémon 1008 ENCOUNTERS" video.
- ↑ In Gen 6, all Mega Evolutions are named "Mega Evolution" in the Pokédex, even for Pokémon with multiple. Starting in Gen 7, they are named after the specific Pokémon (e.g. "Mega Alakazam"), though other sources used these names even before Gen 7. On the Japanese Pokédex website, these names are used in place of the species name for Mega Evolutions, unlike other forms that have their form names separated from their species names (for example, Venusaur's alternate form entries are called "Mega Venusaur" and "Venusaur (Gigantamax)"). In ZA, Tatsugiri and Magearna have secondary form names for their Mega Evolutions that are shown in the Mega Evolution Pokédex; in the regular Pokédex, Tatsugiri's Mega Evolutions use these names (Curly Form, Droopy Form, and Stretchy Form) rather than "Mega Tatsugiri".
- ↑ Urshifu's Gigantamax forms are both called "Gigantamax" in the Pokédex. Other sources specify "Single Strike Style" and "Rapid Strike Style".
- ↑ In SMUSUM, every cap form shares the same Pokédex entry, which is different from the regular form. Starting in SwSh, each cap form has its own different entry.
- ↑ All three Paldean Tauros breeds are called "Paldean Form" in the Pokédex. Other sources give the names "Combat Breed", "Blaze Breed", and "Aqua Breed".
- ↑ 5.00 5.01 5.02 5.03 5.04 5.05 5.06 5.07 5.08 5.09 5.10 5.11 5.12 5.13 5.14 5.15 5.16 5.17 5.18 5.19 5.20 5.21 5.22 5.23 5.24 5.25 5.26 5.27 5.28 5.29 5.30 5.31 The official website for the 30th anniversary logos lists this Pokémon's form name, despite its other forms not being included.
- ↑ All Unown forms are called "One form" in the Pokédex. Some other sources give them all distinct names.
- ↑ The default forms are only given form names ("Kyogre" and "Groudon") in games where the Primal Reversion forms are also included. On the Japanese Pokédex website, "Primal Kyogre" and "Primal Groudon" are used in place of the species name like with Mega Evolutions, unlike other forms that have their form names separated from their species names.
- ↑ 8.0 8.1 This Pokémon's forms have never been able to be registered to the Pokédex in a core series game in which any forms have different Pokédex entries (i.e. Gen 7 onward except BDSP).
- ↑ Arceus's forms are not included in the Pokédex prior to ORAS. All of its forms are called "Arceus".
- ↑ 10.0 10.1 10.2 10.3 Except in games where all gender differences are given form names (Gen 4-6 and ZA).
- ↑ Galarian Darmanitan's Standard Mode is called "Galarian Form" in the Pokédex, while its Zen Mode is called "Zen Mode" (the same name as regular Darmanitan's Zen Mode). Other sources also use the "Standard Mode" and "Galarian Form" terms respectively for these forms.
- ↑ Although regular Darmanitan's Zen Mode has official art, it is not used on official Pokédex websites, which instead use a render of its model. Galarian Darmanitan's Zen Mode only has a render of its model as its official art.
- ↑ Despite Darmanitan's Zen Mode having a separate entry on the Japanese website, Darmanitan's entry also features a note mentioning the form, as is usually done for Pokémon whose forms do not have separate entries.
- ↑ Genesect's forms are not included in the Pokédex prior to ORAS. Prior to ZA, all of its forms are called "Genesect"; in ZA, the default form has its name removed and the four drive forms are renamed.
- ↑ Greninja has never been able to be registered in the Pokédex in a core series game featuring Ash-Greninja. As such, Ash-Greninja has no Pokédex entries in HOME. The official Pokédex website does give it different entries, but these are just reused entries for regular Greninja taken from different games.
- ↑ Vivillon's primary official art only depicts the Meadow Pattern, but all 20 forms have front-facing art that is sometimes used in the same contexts, such as on the English Pokédex website.
- ↑ The English website includes 8 of Vivillon's 20 forms: Meadow Pattern, Continental Pattern, Garden Pattern, Elegant Pattern, Marine Pattern, High Plains Pattern, River Pattern, and Fancy Pattern.
- ↑ The Lumiose Pokédex in Pokémon HOME displays only 3 of Vivillon's 20 forms: Garden Pattern, Meadow Pattern, and Marine Pattern.
- ↑ Poké Ball Pattern is excluded. Fancy Pattern is included but not given a form name.
- ↑ Furfrou's forms are not included in the Pokédex prior to ORAS.
- ↑ Besides Natural Form, only Furfrou's Heart Trim, Star Trim, and Diamond Trim have official art.
- ↑ In addition to the Natural Form, the English website includes 3 of Furfrou's 9 trims: Heart Trim, Star Trim, and Diamond Trim.
- ↑ 23.0 23.1 23.2 23.3 When requesting this Pokémon on the GTS in HOME, specifying its gender automatically specifies its form to match, and vice versa.
- ↑ 24.0 24.1 24.2 All forms share the same official art.
- ↑ The forms' different menu sprites are currently only in HOME and Champions as the Pokémon has not appeared in a core series game that uses them.
- ↑ Neutral Mode is only shown in the collages towards the end of the video.
- ↑ The Lumiose Pokédex in Pokémon HOME only displays Zygarde's 10% Forme and 50% Forme.
- ↑ In SMUSUM, every form shares the same Pokédex entry except Type: Normal. Starting in SwSh, each form has its own different entry.
- ↑ 29.0 29.1 The Japanese Pokédex website lists the form name for this Pokémon's main form, despite its other form(s) not having their own entries.
- ↑ In the Pokédex in SV, Minior has seven forms called "Meteor Form", corresponding to each of its core colors. The entries of all seven are identical.
- ↑ The Pokémon Guidebook originally mentioned Magearna's Original Color form in a note rather than giving it its own entry. In the version 4.0.0 update, the note was removed and the Original Color form was given a separate entry (along with Mega Magearna's Original Color form).
- ↑ Prior to SV, Sinistea and Polteageist's forms are included in the Pokédex but not given names. Other sources used the "Phony Form" and "Antique Form" names for them even before SV.
- ↑ Only the 9 different cream types have different form names in the Pokédex. Every form of a single cream type shares the same name regardless of sweet type.
- ↑ The English Pokédex website lists the form name for this Pokémon's main form, despite its other forms not being included.
- ↑ 35.0 35.1 35.2 35.3 This Pokémon's alternate form(s) use a render of their model as their official art on official websites.
- ↑ There has not been a core series game in which both of Ursaluna's forms can be registered in the Pokédex in the same game. However, the Pokédex in HOME and official Pokédex websites separate Ursaluna's entries by form (with its LA entry for its regular form and its SV entries for its Bloodmoon form).
- ↑ Tatsugiri's Curly Form, Droopy Form, and Stretchy Form all have official art. Mega Tatsugiri's Stretchy Form has official art, while its Curly Form and Droopy Form instead officially use renders of their models as their official art.
- ↑ Tatsugiri's Curly Form, Droopy Form, and Stretchy Form all have their own entries in the Pokémon Guidebook. Mega Tatsugiri only has an entry for its Stretchy Form, but its other two forms are mentioned in a note.
- ↑ Terapagos's Stellar Form uses its render from HOME as its official art on official websites.

- The poster for Pokémon Black Version 2 & Pokémon White Version 2: The Official National Pokédex does not include any alternate forms.
- The Diamond and Pearl poster and the X and Y poster have all of the included forms in a single entry per species for every species. The HeartGold and SoulSilver poster, the Sun and Moon poster, and the Sword and Shield poster have separated entries for all included forms. The Black and White poster and the Omega Ruby and Alpha Sapphire poster have a mix of both.
- The X and Y poster features Mega Evolutions in a separate section at the bottom, while the Omega Ruby and Alpha Sapphire poster and the Sun and Moon poster instead feature them alongside their regular forms like with other forms. The Sword and Shield poster features Gigantamax forms in a separate section at the bottom.
- The Sun and Moon poster does not show the original forms of Pokémon with Alolan forms. The Sword and Shield poster does not show the original forms of Pokémon with Galarian forms, with the exceptions of Meowth, Yamask, and Mr. Mime (the Pokémon whose original forms were obtainable at release in Sword and Shield).
- The Sun and Moon poster shows the form names for Wishiwashi, Silvally, Minior, Mimikyu, Shellos, Gastrodon, and all Alolan forms, despite their other forms not being included.
- The Sword and Shield poster shows the form names for Alcremie, Pumpkaboo, Gourgeist, Shellos, Gastrodon, Mimikyu, Eiscue, Silvally, and all Galarian forms, despite their other forms not being included. Conversely, Rotom's default form does not have its form name shown, despite its other forms being included.
- The only Unown form shown on these posters is the F form.
- The posters that only feature one Cherrim form use the Sunshine Form. However, posters that include both forms list the Overcast Form before the Sunshine Form, as the Pokédex does.
- The Sun and Moon poster and the Sword and Shield poster only include the East Sea forms of Shellos and Gastrodon, despite the West Sea forms usually being listed first.
- Gigantamax Toxtricity is excluded from the Sword and Shield poster as it was yet to be officially revealed at the time.
Form order inconsistencies
Some Pokémon have their forms ordered inconsistently, or are not consistent regarding which form is considered the "primary" form.
- For Pokémon that have both Mega Evolutions and Gigantamax forms, the Pokédex and Pokémon Guidebook in HOME list the Gigantamax form before the Mega Evolution(s), while the Japanese and English Pokédex websites list the Mega Evolution(s) before the Gigantamax form.
- Although regional forms are usually listed after the Pokémon's original form, some material for certain games instead prioritizes regional forms corresponding to that game's region. For example, in the regional Pokédexes in HOME, the Galar Pokédex, Isle of Armor Pokédex, and Crown Tundra Pokédex show Galarian forms first, the Hisui Pokédex shows Hisuian forms first, and the Paldea Pokédex shows Paldean forms.
- The Pokédex and Pokémon Guidebook in HOME list Meowth's Gigantamax form before its regional forms, while the Pokédex in Sword and Shield and the Japanese and English Pokédex websites list it after the regional forms.
- While not an inconsistency per se, Raichu's Alolan form is listed before its Mega Evolutions in the Pokédex and Pokémon Guidebook in HOME and the Japanese and English Pokédex websites, while Slowbro's Galarian form is listed after its Mega Evolution in the same sources.
- Pikachu's Gigantamax form is listed before its cap forms in the Pokédex in HOME, but after them in the Pokédex in Sword and Shield.
- Although Unown's A form is listed first in the Pokédex, its F form is also sometimes treated as its primary form, being the form shown in its official artwork (and thus the one shown on the Japanese and English Pokédex websites, the Pokémon Guidebook in HOME, and official guidebook posters) and its 30th anniversary logo. In the regional Pokédexes in HOME, the Sinnoh Pokédex shows the A form while the Hisui Pokédex and Pokémon GO Pokédex show the F form.
- Burmy and Wormadam's forms are usually in the order Plant Cloak, Sandy Cloak, Trash Cloak, but the Hisui Pokédex in HOME is an exception, showing them in the order Plant Cloak, Trash Cloak, Sandy Cloak.
- Cherrim's 30th anniversary logo uses the Sunshine Form, despite Overcast Form being listed first in the Pokédex.
- Although Shellos and Gastrodon's West Sea forms are listed first in the Pokédex, some material for Sun and Moon, Ultra Sun and Ultra Moon, and Sword and Shield shows only or primarily the East Sea forms, such as official guidebooks and the Galar Pokédex in HOME.
- Although Vivillon's Icy Snow Pattern is listed first in the Pokédex, its Meadow Pattern is usually treated as its primary form, being the form shown in its main official artwork (and thus the one shown on the Japanese Pokédex website and the Pokémon Guidebook in HOME, as well as being listed first on the English Pokédex website) and its 30th anniversary logo. In the regional Pokédexes in HOME, the Paldea Pokédex shows the Fancy Pattern while the Pokémon GO Pokédex shows the Meadow Pattern; the Lumiose Pokédex uses the Meadow Pattern as the primary form, but also displays the Garden and Marine Patterns.
- Although Xerneas's Neutral Mode is listed first in the Pokédex, its Active Mode is usually treated as its primary form, being the form shown in its official artwork (and thus the one shown on the Japanese and English Pokédex websites, the Pokémon Guidebook in HOME, and official guidebook posters) and its 30th anniversary logo. However, the Pokémon GO Pokédex and Lumiose Pokédex in HOME show Neutral Mode.
- In the Pokédex in HOME, the Japanese and English Pokédex websites, the Pokémon Guidebook in HOME, and the 30th anniversary logos website, Zygarde's forms are listed in the order 50% Forme, 10% Forme, Complete Forme; the HOME GTS request menu also lists 50% Forme before 10% Forme. In the Pokédex in Sun and Moon, Ultra Sun and Ultra Moon, Bank, and Legends: Z-A, they are listed in the order 10% Forme, 50% Forme, Complete Forme; the Lumiose Pokédex in HOME also shows 10% Forme before 50% Forme. The Pokémon GO Pokédex in HOME shows the 50% Forme.
- In the Pokédex in Sun and Moon and Ultra Sun and Ultra Moon, Oricorio's forms are listed in the order Pom-Pom Style, Pa'u Style, Baile Style, Sensu Style. In most other sources, they are listed in the order Baile Style, Pom-Pom Style, Pa'u Style, Sensu Style.
- Although Toxtricity's Amped Form is usually listed before its Low Key Form, the English Pokédex website is an exception, as it lists the Low Key Form first.
- In sources that recognize only one Gigantamax form for Toxtricity (the Pokédex in Sword and Shield, the Pokédex in the Nintendo Switch version of HOME, the Pokémon Guidebook in HOME, and the English Pokédex website), it is listed after both the Amped Form and Low Key Form. In the Pokédex in the mobile version of HOME, the order is Amped Form, Gigantamax (Amped Form), Low Key Form, Gigantamax (Low Key Form). On the Japanese Pokédex website, the order is Amped Form, Low Key Form, Gigantamax (Amped Form), Gigantamax (Low Key Form).
- Zacian and Zamazenta's 30th anniversary logos use the Crowned Sword and Crowned Shield forms respectively, despite the Hero of Many Battles forms being listed first in the Pokédex.
- In the Pokédex and Pokémon Guidebook in HOME, Urshifu's forms are listed in the order Single Strike Style, Gigantamax (Single Strike Style), Rapid Strike Style, Gigantamax (Rapid Strike Style). In the Pokédex in Sword and Shield and the Japanese and English Pokédex websites, they are listed in the order Single Strike Style, Rapid Strike Style, Gigantamax (Single Strike Style), Gigantamax (Rapid Strike Style).
- Maushold's official artwork shows its Family of Four form, which is considered the primary form in the Pokémon Guidebook in HOME, the Paldea Pokédex in HOME, and the English Pokédex website. Meanwhile, Family of Three is considered the primary form in the Pokédex in Scarlet and Violet, the Pokédex in HOME, the Kitakami Pokédex and Pokémon GO Pokédex in HOME, the GTS request menu in HOME, the Japanese Pokédex website, and its 30th anniversary logo.
- Palafin's 30th anniversary logo uses the Hero Form, despite Zero Form being listed first in the Pokédex.
Official Pokédex websites
Other Asian Pokédex websites seem to be based on the Japanese website, matching it in terms of images used, forms featured, and form names used. Unlike the other Asian Pokédex websites, the Korean one does not feature notes describing forms for Pokémon whose forms lack their own separate entries. Additionally, the quirk of the Japanese website where Darmanitan's Zen Mode is described in a note despite having a separate entry is not present on the other Asian websites, which lack the note.

The Asian English Pokédex websites refer to Alolan forms as "Alola Form", matching the Pokédex in Gen 7, while the western English Pokédex websites instead use "Alolan Form", matching the Pokédex in Gen 8 onwards. This discrepancy does not exist with other regional forms, which are consistently referred to as "Galarian Form", "Hisuian Form", and "Paldean Form".
