Primal Groudon and Primal Kyogre

Primal Reversion (Japanese: ゲンシカイキ Primal Regression) is an ancient transformation that debuted in Omega Ruby and Alpha Sapphire. Primal Reversions are identified by having "Primal" in front of their name. This transformation is similar to Mega Evolution, expect the Pokémon goes through a devolution, reverting to the Pokémon's Primal state. The Pokémon who goes through the Primal Reversion are said to regain their true powers.

Primal Reversion's Origin

According to the Hoenn legend, it was a during the primal age, early in the world’s history. The natural world was overflowing with energy. That energy granted Groudon and Kyogre an overwhelming power. Seeking ever more of that energy for themselves, the two clashed again and again, and their battles cast the people and Pokémon of Hoenn into great danger. Brimming with the overwhelming power granted to them by nature’s energy, their transformation was named "Primal Reversion" by later people. And the people called their tormentors Primal Groudon and Primal Kyogre.

One Pokémon looked down upon this battle from the sky above. The Pokémon that dances in the heavens: Rayquaza. Rayquaza is venerated by the people of Hoenn as their protector and savior. When Primal Groudon and Primal Kyogre had battled for 20 dawns and 20 nights, the world began to lose its natural balance and tilt toward destruction. The people, unable to endure any more, cried out to their mighty savior Rayquaza before a great stone that shone with rainbow brilliance. And a change came upon Rayquaza, weaving in the sky above Primal Groudon and Primal Kyogre. Suffused in brilliant light, its body began to transform visibly. And Mega Rayquaza appears who then confronted Primal Groudon & Primal Kyogre and created a terrible wind that sapped their power, causing the two to vanish.

Primal Reversion

To do this, Groudon and Kyogre has to be holding a special item. For Groudon its the Red Orb and forKyogre its the Blue Orb. As soon as Groudon/Kyogre is sent out in battle, it will become its Primal form. Unlike Mega Evolutions, there are no limits as to how many you can have on a team (barring limits on the amount of Primal Reversions) and you can have them on the same team as an active Mega Evolution.
