This template is to be used for Pokémon that can swap between different forms.

Template fields
Fields relating to the overall display
- type1-disp - The type to be used for the background color of the evobox.
- type2-disp - The type to be used for the border color of the evobox. If not provided, falls back on type1-disp. If that's not provided either, falls back on the typing of the Pokémon's base form.
- name1 - The Pokémon's species name.
Fields relating to the Pokémon's base form (form 1):
- sprite1 - The file name of the sprite/art for form 1.
- type1-1 - Form 1's primary type.
- type2-1 - Form 1's secondary type.
- forme1 - Form 1's name.
- loc1 - Location requirement to change into form 1.
Fields relating to the Pokémon's second form (form 2):
- item1 - Item requirement to change into form 2.
- itemfile1 - A custom file name, if "Bag item1 Sprite.png" is not the desired file.
- itemsize1 - A custom display size for item1's image. Defaults to 24px.
- itemlink1 - A custom page to link to, if item1 is not the desired target page.
- itemdisplay1 - A custom item name to display, if item1 is not the desired display name.
- sprite2 - The file name of the sprite/art for form 2.
- type1-2 - Form 2's primary type.
- type2-2 - Form 2's secondary type.
- forme2 - Form 2's name.
- loc2 - Location requirement to change into form 2.
Fields relating to the Pokémon's third form (form 3):
- item2 - Item requirement to change into form 3.
- itemfile2 - A custom file name, if "Bag item2 Sprite.png" is not the desired file.
- itemsize2 - A custom display size for item2's image. Defaults to 24px.
- itemlink2 - A custom page to link to, if item2 is not the desired target page.
- itemdisplay2 - A custom item name to display, if item2 is not the desired display name.
- sprite3 - The file name of the sprite/art for form 3.
- type1-3 - Form 3's primary type.

Note that the fields type1-disp, name1, sprite(x), type1-(x), type2-(x), forme(x), and loc(x) will be passed to Template:Evobox/Setup.
