Cleanup and scope reduction
Based on my understanding of this template, the parameters are:
1 - national dex number + form indicator, which get passed into Template:MS
2 - link text should point to
3 - text to display
4 - rarity
5 - whether the entity is a Pokémon
6 - form (?)
In my opinion, some of these parameters could be removed. I don't see why this template needs to be responsible for listing and linking the form name; that can be something the template user does (e.g. see how Sunny Castform is linked in Fire (type)#Pure Fire-Type Pokémon). I would argue that parameter 5 could also be removed. There is currently one place it is being used--the vocab list for the Shift Vocabulary Challenge--and I believe that this template should not be used for that case. A separate template, or even a simple table, could be used for vocab lists. It's not worth trying to make this one template do multiple things.
These changes, plus naming some parameters and re-formatting of the source, would make this template much more readable and purposeful. I would imagine usage would then look something like:
