The first one is the Pokémon's number.
The second is is the sprite is a gif or png. Gif is set as the default.
The third is the Pokémon's name.
The fourth is for the type of its Poké Assist.
The fifth is the field move's name.
The sixth is the number of the field move. Add the x if needed.Leave empty if inapplicable.
Add yes in the seventh to be able to add notes.
The eight is for optional notes.
