Four Island is the most isolated of the seven cities of the Sevii Islands, as, quite literally, there are no routes connecting to it.
The city is notable for being the home of Lorelei, the first of the Kanto Elite Four during Generation I and Generation III. It is very possible that this city is where one could find her during Generation II, if it was able to be reached at that time.
Four Island is also home to the breeding center of FireRed and LeafGreen, as the one that is on Route 5 only has room for one Pokémon. Because of this, it is possible for three Pokémon to be raised at once in these games, something that has never happened before in a Pokémon game.
The background music used in Four Island is the same as that of Azalea Town and Blackthorn City of Johto.
