Professor Carolina (Japanese: カラシナ博士 Dr. Karashina) is a professor that appears in both the games and anime, she also appears in the manga but is not referred by name. She is also Cynthia's grandmother.
In the games
Cynthia does have a grandmother in Diamond, Pearl, and Platinum; however, she is not referred to by name. The player is given an Old Charm by Cynthia to deliver to her grandmother. Once the player arrives in Celestic Town, the grandmother tells the story of the lake guardians.
In the games, once the player has faced Dialga or Palkia, she will show a picture of Dialga or Palkia, depending on which version is played. This only registers the Pokémon as "seen" in the Pokédex and does not add an entry. In Platinum, as witnessing the events at the Spear Pillar adds both Dialga and Palkia to the Pokédex, she will instead inform the player of their ability to catch Dialga and Palkia if they own their respective orbs.
In the anime
She made her debut appearance in Losing Its Lustrous!. She was the head of the Celestic Town Historical Research Center which originally contained the Lustrous Orb before the events in Losing Its Lustrous! and Double Team Turnover! which led to Team Galactic obtaining the orb.
She is a very outspoken lady and even directly asked Ash to battle her granddaughter, Cynthia. She was shown to be very proud of her granddaughter's status as Champion, telling Ash that he would have to train a lot harder if he wanted to beat Cynthia.
She appeared again in Saving the World From Ruins!, studying the connection between the Iron Island ruins and the Veilstone City meteorites.
Her latest appearance was in The Needs of the Three!, where she was at Lake Acuity, working with Professor Rowan and Gary to keep the three Lake Guardians safe.

In the manga
In the Pokémon Adventures manga
Cynthia's grandmother has appeared in the Pokémon Adventures manga. She loves Moo-Moo Milk and often visits Café Cabin to savor some. She led Diamond, Pearl and Platinum to Celestic Town in Problematic Probopass and Mad Magnezone I, telling them the legend of Sinnoh's three lakes and the Lake guardians on the way. In Platinum arc it turns out that she was the one that created the meteor-filled craters in Veilstone City.

Cynthia's grandmother has appeared in the Pokémon Diamond and Pearl Adventure! manga in a role similar to her in the games. She is first shown trying to expel a member of Team Galactic from Celestic Town.
