The Moon Stone (Japanese: つきのいし Moon Stone) is a type of Evolution stone introduced in Generation I.

If thrown, it will deal 1 damage (2 if sticky). Using this item in a dungeon will consume it, with no effect.
In Pokémon Mystery Dungeon: Red Rescue Team and Blue Rescue Team, if the player offers a Moon Stone from the Team Toolbox when attempting to evolve in Luminous Cave, Nidorina, Nidorino, Clefairy, Jigglypuff, and Skitty can be evolved into Nidoqueen, Nidoking, Clefable, Wigglytuff, and Delcatty, respectively. It costs 3000 Poké and can be sold for 1 Poké.
In Pokémon Mystery Dungeon: Explorers of Time, Explorers of Darkness, and Explorers of Sky, if the player offers a Moon Stone at Luminous Spring, Nidorina, Nidorino, Clefairy, Jigglypuff, and Skitty can be evolved into Nidoqueen, Nidoking, Clefable, Wigglytuff, and Delcatty, respectively. It costs 1000 Poké and can be sold for 1 Poké.

In Pokémon Conquest, Jigglypuff and Munna can evolve into Wigglytuff and Musharna, respectively, if their link is improved while their Warrior is equipped with a Moon Stone.
A Moon Stone can be purchased from the traveling merchant for 5000 gold, and sold for 2500-3250 gold.

In the anime
In Clefairy and the Moon Stone, a giant Moon Stone appeared deep within Mt. Moon, being worshipped by a group of Clefairy who lived there. It was soon stolen by Team Rocket, and ultimately ended up being blown up; the shards from the shattered giant Moon Stone rained down on the Clefairy and caused some of them to evolve into Clefable. In this episode, the characters speculated that the Moon Stone was how various Pokémon—Clefairy in particular—arrived in the Pokémon world.
A Moon Stone appeared in a flashback in Delcatty Got Your Tongue?, evolving Dr. Abby's Skitty into a Delcatty.
Mystery on a Deserted Island! featured the Moon Stone along with all of the other Evolution stones available as of Generation V.
A full set of Evolution stones was seen on display in a stone shop in Geosenge Town in The Cave of Trials!.

In The Electric Tale of Pikachu manga
In Clefairy Tale, Ash, Professor Oak, and Bill witnessed a Clefairy evolution ceremony involving a large Moon Stone.
The focus of the chapter To Evolve or Not to Evolve, That Is the Question! was Evolution stones, including the Moon Stone.
In the Pokémon Adventures manga
A Moon Stone was first mentioned by Professor Oak in Gyarados Splashes In! as a rock with a crescent moon-shaped indentation that boosts a Pokémon's power immensely. In the next round, Team Rocket searched for one in Mt. Moon. However, it was Red who eventually found it. In The Winged Legends, Red used his Moon Stone to evolve Green's Clefairy into a Clefable in order to fight Thu-Fi-Zer during the Silph Co. showdown with Team Rocket.
In Guile from Mawile, Ruby's Skitty evolved into a Delcatty due to being exposed to one of Steven's Moon Stones.
In Give It Your Best, Blastoise, during the battle against the Deoxys Duplicates, Green evolved her Jiggly and Nido into Wigglytuff and Nidoqueen, respectively, using two Moon Stones.
In Homecoming, it was revealed that Black's Musha evolved into a Musharna using a Moon Stone received from Caitlin.

In the TCG

The Moon Stone was released as a Trainer card in the Pokémon Trading Card Game during the Japanese Original Era with Extended Sheet 1. The card was only ever released in Japanese and has never been reprinted. It allows the player to add 1 Colorless Evolution card from their deck to their hand.
Unrelated to the other TCG card, a Moon Stone appears as a held item on the Secret Wonders Clefairy card. It allows Clefairy to evolve on the turn it is played.
Trivia
- From Generation IV onward, the Moon Ball is more likely to catch Pokémon that evolve via Moon Stone. While this was intended to be the case in Generation II, the Poké Ball always has 1× effectiveness due to a bug.
- Through a glitch in the Generation I games, Pokémon that would normally require an Evolution stone to evolve can be evolved without it, provided that the Pokémon levels up in the battle and the player has sent out a specific Pokémon in the same battle. In the case of the Moon Stone, this Pokémon is Exeggutor.
- The Moon Stone shares its name with an actual gemstone.
